Join the CSSSJ!

The CSSSJ membership application form can be used either by new members or for membership renewal.

To apply for membership, fill in all of the necessary fields in the form, then click the "Submit Application" button at the bottom of the form to submit your application electronically to the CSSSJ membership director. Shortly afterward, you will receive an email from the membership director containing an address to which you must send a check made payable to "CSSSJ" in the amount shown in step 2 of the form. Your application is not complete until your check is received and processed.

PLEASE READ: CSSSJ membership is good until December 31st of the year in which dues are paid.

  1. Select whether you are a new or renewing member.
     Membership Type:  New Member    Renewal

  2. Select whether you are paying for a single or couples membership, or if you are a life member.
     Dues:   Single Membership ($20)     Couple's Membership ($25)     Life Member

  3. Type your first and last name, and for couples memberships, the first and last name of the additional member, in the following boxes.

  5. Select whether or not you want your name and address to appear in the CSSSJ membership directory. (The membership directory is published yearly and provided to all club members.)

  6. Type any additional comments or instructions for the CSSSJ membership director in the box below:


  7. Carefully review all of the information above for accuracy, then, click the "Submit Application" button.

      

    Clicking the "Reset Form" button resets all information in the form.
